SKS Skateboarding Training Waiver Form

Event Details: Skateboarding Training Programme, including 1v1 Private Lesson and 1V5 Group Lesson

In consideration of being allowed to participate in the skateboarding training event organized by SKS SKATE, I, the undersigned participant or parent/legal guardian of the participant if the participant is a minor, hereby acknowledge and agree to the following terms and conditions:

Assumption of Risks:

I understand that skateboarding involves certain inherent risks, including but not limited to falls, collisions, injuries and death.

I am aware of the dangers and risks associated with skateboarding and voluntarily choose to participate, assuming all risks involved.

Health and Fitness:

I represent that I am physically and mentally fit to participate in the skateboarding training event.

I will notify the organizers of any pre-existing medical conditions or injuries that may affect my ability to participate safely.

I authorize and consent to emergency medical treatment or hospital care rendered to me or my child by any licensed medical professional.

Protective Gear:

I acknowledge that wearing proper protective gear, including a helmet, knee pads, elbow pads, and wrist guards, is mandatory during the skateboarding training event.

I understand that failure to wear proper protective gear may result in denial of participation.

It is my responsibility to check and maintain the safety condition of all equipment used while participating in any skateboarding activities including personal, rented, and borrowed gear.

Compliance with Rules and Instructions:

I agree to comply with all rules, regulations, and instructions provided by the organizers and instructors during the skateboarding training event.

I understand that failure to adhere to these instructions may result in my removal from the event.

Release and Waiver of Liability:

I hereby release, waive, and discharge SKS SKATE, its staff, instructors, volunteers, sponsors, and affiliates from any and all liability for any injury, loss, damage, or claim that may arise during or as a result of my participation in the skateboarding training event.

This release and waiver of liability include, but are not limited to, any claims based on negligence, premises liability, or breach of warranty.

Photographic and Video Release:

I grant SKS SKATE the right to take photographs and videos of me during the skateboarding training event.

I authorize SKS SKATE to use and publish these images for promotional, educational, and archival purposes.

I have read this waiver form carefully, fully understand its contents, and voluntarily agree to its terms.
